I recently volunteered to help a small church start a library with the 800 or so books they had donated. I was hoping someone could send me the template or form that they have patrons fill out before they are allowed to check out books. Thank you

This may not work as a template, but it is what we use.

I can't send a template but we use a 3x5 or 4x6 card with the person's name, last and first, address, city, state, zip code,
phone number, email, and names & ages of family members.
