Our library is not automated at this time. I need accession sheets for DVD's and other AV items. I have not been able to find them. They used to greenish a long time ago. I found acc. sheets at LifeWay online. Where can I order the AV sheets?

Mary, if you have not been able to find the accession sheets for audiovisuals, you can generate a form yourself in Word or Excell. I will list the columns which are on the last av forms that we bought several years ago.
On the title line is written "Audiovisual Accession Sheet for _____________" and you fill in the blank for each audiovisual format. Use a different sheet for each type so that you will be able to know how many of each that you have.
The columns are as follows: Date;No.; Title; No. Items; Producer; Year; Source; Cost; Remarks
Date is date entered; No. starts with number 1, and continues consecutively for each format; Title; No. items (for example: number of CDs in an audio book); producer; Year (of production); Cost: retail, not discounted; Remarks: use for memorial, gift, etc or "Removed"
